Responsibilities
The Registered Home Health Nurse provides intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities. The Home Health Visiting Nurse is responsible to communicate the patient’s progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.
Registered Nurse (RN)/ Home Health Nurse/Wound Care Nurse Responsibilities:
- Admits patients eligible for home care within 24–48 hours under physician’s order and completes all required documentation per agency policy
- Performs comprehensive assessments to identify patient needs, establish goals, and develop individualized care plans
- Provides skilled nursing services including evaluation, procedures, teaching, and training as outlined in the Plan of Care
- Updates care plans and resolve patient problems as conditions change or at recertification
- Communicates patient status and interdisciplinary needs to Clinical Supervisor, referring physician, and follow-up clinician
- Submits documentation including visit notes, communication logs, change orders, and supervisory notes per agency schedule
- Conducts OASIS assessments and submits recertification paperwork in compliance with Medicare and agency standards
- Supervises Home Health Aides and documents per Medicare and agency requirements
- Participates in team activities including staff meetings, educational in-services, discharge planning, and process improvement initiatives
- Serves as a preceptor for new nursing staff during orientation as requested
- Delivers Age-Friendly Care by incorporating the 4Ms framework: What Matters, Medications, Mentation, and Mobility
- Meets physical demands of the role including lifting up to 40 pounds, driving 2–4 hours daily, and performing repetitive movements
Qualifications
Registered Nurse (RN)/ Home Health Nurse/Wound Care Nurse Required Knowledge, Skills and Experience:
- Current unencumbered State professional Nurse License
- Must maintain a valid driver’s license and good driving record
- The ability to make sound professional clinical judgment
- The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
- Proficient clinical skills
- Excellent verbal and written communication skills
- Proficiency in personal computer use, including e-mail, clinical, word processing, spreadsheet and presentation software
- Active CPR Certification
Preferred Knowledge, Skills and Experience
- One year of experience as a home care professional nurse and is competent in performing home care comprehensive assessment
